Our Estate Planning Services are billed at a flat fee rate of $250 for an individual plan, $500 for a couple. Extra Estate Planning Services are billed at an hourly rate of $75 per hour.

Our Estate Planning Services:
- Statutory Durable Power of Attorney
- Medical Power of Attorney
- Directive to Physicians (also know as “Living Will”)
- HIPAA Authorization
- Declaration of Guardian for you and your minor children
Extra Estate Planning Services:
- Last Will and Testament
- Do-Not-Resuscitate Order
- Disposition of Remains
- Agent to Control Disposition of Remains
- Declaration for Mental Health Treatment
- Hospital Visitation Authorization (for unmarried couples)
- Notary Public services are available for a fee of $6 per acknowledgment, plus mileage.
